Grant-Winning Communities

New Haven, CT
Following the selection of New Haven, CT for our first grant, medical professionals in the area are now eligible for up to $3 million in granted software. 

In the time since the community's selection, we have completed the solution design process to ensure that the products implemented in the area deliver the results sought by the area's healthcare professionals.  We have finalized the products Hospital of Saint Raphael will host for the community -- Misys Connect to facilitate data exchange between providers as they begin using electronic medical records, and the hosted option of both practice management and electronic medical record products. 

In recent months, we have begun working with the individual organizations that applied for a grant to scope their software needs and how they fit into the larger community network.  Grant contracting is underway, and the first implementations will begin this month. 

Tampa, FL
On November 1, 2007, the Center for Community Health Leadership announced the winner of the second $3 million grant: Tampa Bay, FL.  Tampa Bay area as a Center grant recipient because of the area’s reputation for healthcare excellence and their commitment to making the idea of community data sharing a reality.

Through January, physician practices, hospitals and homecare agencies in the area were encouraged to apply for their portion of the grant, specifying the software that they were requesting via the program. The results were overwhelmingly positive, resulting in applications from 250 physician practice locations, representing 888 healthcare providers who deliver care to almost three million Tampa Bay residents! 

 

Implementation and the community connectivity efforts kicked off in Spring 2008 and will continue through early 2009.


 

Vision Recognition Grant Communities

In recognition of community connectivity efforts already underway and as part of an initiative to expand the number of physicians able to share data with one another, the Center for Community Health Leadership has set aside grants of several hundred thousand dollars in electronic health record software for medical professionals in each of the following areas: Elmira, NY; Rocky Mount, NC; Hattiesburg, MS; and Silicon Valley, CA.

The local hospitals in those communities - Arnot Health, Nash Health Care System, Forrest General Hospital and Daughters of Charity Health System - have all taken leadership positions in the area of health information exchange and connectivity between local medical professionals.  The grants from the Center are designed to help those communities expand upon the work already completed by expanding the number of physicians with access to electronic health records. 

All applications for Elmira, Rocky Mount and Hattiesburg have been accepted and are currently being processed; the application process for interested physician practices, community health centers, health clinics and home care agencies in San Jose, CA will close February 29th, 2008.
