It is only necessary to complete this application if you are seeking a grant of electronic health record software or practice management software.  If you are instead interested in keeping your current systems but ultimately linking in to the connected network of medical professionals that will result from this grant program, you do not need to complete an application.  Instead, please visit www.dr-connect.com, where you will have an opportunity to indicate your desire to be kept informed as to the status of the project and opportunities to connect into the network once the infrastructure work is completed.

To Apply

In the following pages, you will be asked for basic information about your organization that will enable us to assess the software configuration required to bring your clinicians live on electronic health records. Please be prepared to report the following information:

• the size and volume of your organization
• the type(s) of products you are requesting via a grant
• your initial preference regarding accessing the product(s) via a hosted (ASP) model or one where the software is installed within your practice
• the names of the other medical organizations to whom you refer or from whom you are referred the most patients so that we can assess referral patterns within the applicant pool. 

Additionally, in this application you have the opportunity to indicate that you understand the criteria of the grant.

Please note that all applications must be submitted no later than January 18, 2008, to be considered for a grant.  Applications received after January 18, 2008 will be excluded from consideration. (Application deadline was recently extended due to a recent upsurge in the number of applicants and our desire to consider all interested parties.)

Important Note

Completion of the information sought here does not constitute a contract or any binding commitment, either on the part of your practice or the Center for Community Health Leadership. This form is simply necessary for the Center to determine the total value of the grant being requested in your community.

After all information is received, each individual medical organization applying for a grant will have the opportunity to review the Misys Healthcare Systems Master Agreement with all related terms, including financial and legal requirements related to the grant. At that point, all individual medical organizations will have the final choice to participate or not participate in the program.
