Thank you for your interest in a grant from the Center for Community Health Leadership, sponsored by Misys Healthcare Systems, and congratulations on Westwood, NJ's selection for a grant. 

Westwood's inclusion in the Center's special Vision Recognition program is due in part to the thought leading work being done by Pascack Valley Hospital in the area of clinical community connectivity.  As a result, $250,000 in electronic medical record software will be distributed to medical organizations in the area to help launch the development of a networked group of medical professional for the purposes of improving patient care and business efficiency.

In the following pages, you will be asked for basic information about your organization to enable us to assess the software donation required to bring your clinicians live on electronic health records. We will need information about the size and volume of your organization, as well as your current technical capabilities. Additionally, we need to ensure that you understand the criteria of the grant. Please note that all applications must be submitted no later than May 31, 2007, to be considered for a grant.  Applications received after May 31st will be excluded from consideration.

Completion of the information sought here does not constitute a contract or any binding commitment, either on the part of your practice or the Center for Community Health Leadership. This form is simply necessary for the Center to to evaluate your application against the others that are received. After all information is received, the Center will contact those that are selected for further conversation, at which time they will have the opportunity to review the Misys Healthcare Systems Master Agreement with all related terms, including financial and legal requirements related to the grant. Before anything is finalized, the selected individual medical organizations will have the final choice to participate or not participate in the program.
